Patients may register online up to 30 days prior to surgery.

What You will Need

You will need a list of current medications with dosages, allergies and any special needs to complete the forms. If you take blood thinning medications, please contact your physician for direction immediately upon scheduling surgery.

To Register Now:

  2. Print the form and fill it out
  3. Fax the completed form to 484-679-4407 or email it to register@kopsurgerycenter.com
